Order Form — Personal Information
Certain form fields of our order form's second page, as seen in the above screenshot, seem to give the most concern to new customers. Hopefully, this mini tutorial will help answer your questions.
State
We understand that not every country have states. If your country has any division above your city, and that area of your country is listed on the billing address for the credit card you plan to use for billing purposes, please simply enter its name into that form field. If your billing address simply reads:

then please simply put "N/A" (meaning "not applicable"), or a dash (—) in the State form field. We'll understand when we see your billing address' country, and the order form will not respond with an error.
Zip
For this form field, please enter the postal code for the billing address of your credit card. Uniqueness is not a factor; we understand that many cities—and even countries—share exactly the same postal code/postcode/post code/zip code/postal index number. We also understand that some countries do not follow this convention at all. If some form of universal postal number exists for your credit card's billing address, whatever it is, even if it contains letters and numbers, please enter it as shown on your credit card's bill into this portion of the form. If your country does not have any form of such identifying number, and therefore, neither does your credit card's billing address, then please leave this form field blank, or enter five zeroes (00000).
Order Form — Payment Information
Just below the Personal Information portion of the order form, you'll find the form in which to enter your credit card information, as shown:
Card Number
Please enter the entire credit card account number, without spaces. Even if the physical credit card has spaces, please write the entire account number as one continuous list of numbers for this form.
Site5 currently accepts Visa, MasterCard, and American Express credit or debit cards.
We do not accept direct PayPal payments, Discover cards, or off-line payments
(such as checks or money orders, or company purchase orders) at this time.
We also do not accept specialized cards such as Solo/Switch, Laser, Electron or Maestro cards.
Name on Card
Please make certain that you give your name, middle initials and all, exactly as it appears on the physical credit card. It does not matter that your name as given for your Site5 account in the form above isn't exactly the same; that correlation doesn't matter. What is the crucial match is the name on this form as given, and the name as printed on the physical credit card and the credit card's bank statements. So you don't need to alter your Site5 account's name so that it reflects this rendering of your name, or vice versa.
